These documents can also help you find your plan ID.
Where can I find my plan ID?
Check your Medicare Advantage or prescription drug plan card from your insurance company. Look for a code like H1234-001, sometimes printed near the bottom or on the back.
Can’t find it on your card? Check your Annual Notice of Change, Summary of Benefits, Evidence of Coverage, or insurer’s member portal. You can also ask your insurance company or agent for your contract and plan ID.
Your card may look different, and some cards do not show the full plan ID. You do not need to upload a photo of your card.
The red, white, and blue Medicare card shows your personal Medicare number. It does not show the Medicare Advantage or Part D plan ID needed for this lookup.
Data: CMS 2026 and 2027 landscape and 2027 plan crosswalk, released September–October 2026. Always confirm with your Annual Notice of Change.

Use the plan ID on your plan’s Annual Notice of Change or Evidence of Coverage.

Medicare Machine may share the details you send with a licensed insurance agent or agency to respond to your request. Medicare Machine may receive compensation for this referral or the transfer of your request.
A little preparation makes the call easier.
Have these items nearby when you speak with an agent:
- Your Medicare card and your current insurance plan card.
- Your doctors and specialists—names, offices, and locations.
- Your current prescriptions—medicine names, dosages, and how often you take them.
- Your preferred pharmacy and any mail-order pharmacy you use.
- Any plan termination or non-renewal notice to help the agent review enrollment deadlines and possible Medicare Supplement rights.
- Your Annual Notice of Change and any questions about benefits or costs that matter to you.
Keep these items for your conversation. Do not send your Medicare number or medical records through this contact form.
Questions about checking your plan
What does Medicare Machine compare?
Medicare Machine compares selected published 2026 and 2027 Medicare Advantage and Part D plan fields, including premiums, drug deductible, in-network out-of-pocket limit, and selected medical and extra benefits when matching CMS records are available.
Where do I find my plan ID?
Check your Medicare Advantage or prescription drug plan card for a contract and plan ID such as H1234-001. It may be near the bottom or on the back, but card layouts vary and some cards do not show it. You can also find it in your Annual Notice of Change, Summary of Benefits, Evidence of Coverage, or insurer member portal. Do not enter your personal Medicare number or member ID.
Can I check whether my plan is renewing for 2027?
The lookup shows the published CMS plan crosswalk status. If the plan has multiple destinations or the matching information cannot be verified, Medicare Machine asks you to confirm the details in your Annual Notice of Change instead of choosing a destination for you.
Does this show every change to my plan?
No. It is a summary of selected published fields. Confirm covered doctors, prescriptions, benefit limits, network rules, and all other changes in your Annual Notice of Change, Evidence of Coverage, and Summary of Benefits.
Can a licensed insurance agent help me?
You can contact your current agent or submit a contact request directly through Medicare Machine. Your location helps identify an agent or agency serving your area. There is no obligation to enroll.
Is Medicare Machine a government website?
No. Medicare Machine is an independent educational site and is not connected with or endorsed by the U.S. government or Medicare. Medicare.gov provides official Medicare information and plan comparison resources.
If my Medicare Advantage plan ends, can I get a Medicare Supplement?
If your Medicare Advantage plan leaves Medicare or stops serving your area and you return to Original Medicare, you may have guaranteed-issue rights to certain Medicare Supplement (Medigap) policies. These rights can protect you from denial or higher prices because of health conditions. Application deadlines, age, state rules, and policy availability matter. A plan ID change or a discontinued stand-alone Part D plan alone does not establish this right. A licensed, U.S.-based agent can review your notice, confirm your options, and help you assess whether a supplement fits your needs and budget.
